New Patient Registration

The Royal Well Surgery accepts patients wishing to register, who have moved into, or are living in, our catchment area. Our GP practice boundary is within 3 miles of Cheltenham town centre and includes locations such as Shurdington and Bishops Cleeve as well as Cheltenham itself.

Everyone seeking registration will be required to demonstrate that they are entitled to free NHS treatment. In order to register as a patient you will need to complete a registration pack and also provide identification (passport or medical card) and evidence that you permanently live at an address in our area for a settled purpose (utility bill, council letter, bank statement etc.).

Changes to Personal Details

It will help us greatly, if we always have the correct information about our patients. If you change your name, move house or alter your telephone number, please let us know immediately. Incorrect information may result in a home visit to the wrong house, inability to contact you to alter an appointment, or to arrange a follow up if we receive abnormal test results.
