How does Triage work?
To access our triage service for urgent ‘non-routine’ care, call the surgery on 01242 215010 and one of our trained Care Navigators will ask you a few questions regarding your symptoms and complete a triage form for you over the phone.
The triage requests will be passed over to our Duty Team who will then check your request alongside your medical records and notes from previous consultations, to decide the best course of action.
We will respond to you the same day with an outcome. This may be in the form of a face to face appointment, phone call or text message.
After the Clinician has made a decision about the required care, you will be contacted the outcome with any one of the following.
- A same day appointment with a clinician – phone or face to face
- An appointment within the next 1-7 days with a clinician – phone or face to face
- A routine appointment with a clinician – phone or face to face
- A referral to our in-house pharmacy team for medication issues
- A referral to ‘Pharmacy First’ for minor illness that the pharmacy can prescribe and treat to save you time waiting for an appointment
- A text message from the clinician for results, information or sign posting.
